What do you mean by “participating clinic”? 

A participating clinic is a Community Controlled Health Service connected with Deadly Choices, offering a Deadly Choices shirt after completing your 715 Health Check.

How do I use the health check map?

Enter a street name, postcode or a place you know (like a shopping centre). The map will show the closest participating clinics near you.

I recently had my health check. How do I enter?

If your 715 Health Check is current (within the last 9–12 months) and completed at a participating clinic, you’ll be automatically entered into the draw.
Please note, the Deadly Blues Experience is only available through participating clinics in New South Wales.

What is a 715 Health Check?

A 715 Health Check is a free annual health assessment for all Aboriginal and Torres Strait Islander peoples. The appointment will be approximately one hour.
